Preadmission Screening and Resident Review (PASRR) Overview

PASRR is a two-part screening process. There is a Level I screening which may indicate that a Level II assessment is needed. The Level I screening is looking for evidence of a serious mental illness, intellectual disability, or a condition related to intellectual disability. The purpose of a Level I screening is to identify anyone who may need a PASRR Level II evaluation.

The purpose of a Level II evaluation is to confirm that an individual has a serious mental illness, intellectual disability, or a related condition and to assess the need for additional services. ​These services are used to support people while they are also receiving nursing facility services.

Acentra Health works with the State of North Carolina to complete the PASRR Level II evaluations. These assessments will be in-person, at the location of the person being screened and will be conducted by a qualified mental health and/or intellectual disability professional.
