Acentra Health is honored to partner with the North Carolina Department of Health and Human Services, Division of Health Benefits to administer the NCLIFTSS (Linking Individuals and Families to long-term services and supports) program to streamline access and enrollment in Medicaid Direct Long-Term Services and Supports (LTSS). Acentra Health will help enhance quality through increased provider training and technical support, outreach to under-served LTSS eligible beneficiaries, and coordination between LTSS and benefit plans.

Kepro is now Acentra Health

In June of 2023, the North Carolina Department of Health and Human Services announced that Keystone Peer Review Organization (KEPRO), now Acentra Health, was selected by the North Carolina Department of Health and Human Services to deliver assessment and transition services for the NCLIFTSS Program (Linking Individuals and Families to Long Term Support Services). As a result of this award NC Medicaid will expand the existing independent assessment program to include the 1915(c) home and community-based services waivers which will significantly improve coordination of Medicaid Long-Term Services and Supports (LTSS).

This initiative will provide beneficiaries a long-requested single point of entry for accessing Medicaid LTSS and streamline the processes between initial contact and service enrollment. This single point of entry will be branded as NC Medicaid Linking Individuals & Families to Long Term Services and Supports (LIFTSS).

NCLIFTSS is designed to:

  • Streamline access to LTSS 
  • Reduce wait time between initial contact and service enrollment  
  • Enhance quality through increased provider training and technical support  
  • Outreach to under-served LTSS eligible beneficiaries to address health disparities 
  • Improve coordination between LTSS and benefit plans

As the state contractor for NCLIFTSS, Acentra will conduct intakes, assessments, and other screenings to determine eligibility for the following LTSS programs:

In addition, the selected vendor will also provide management and oversight for the Pre-admission Screening and Resident Review Process (PASRR) Level II and serve as North Carolina’s Transition of Care Coordination (TCC)  for interested Skilled Nursing Facility (SNF) residents, with options for transitioning from institutional care to community-based living as well as those transitioning out of managed care.  

This contract will also support transition of care for LTSS members moving in and out of managed care.
